(Physician, Dietitian and Nurse)

This package uses the same team-based, multi-disciplinary approach as with all other packages offered at La Vie.  Each client will be seen initially by the team Registered Nurse, Physician as well as the Dietitian.  Following your initial assessment we will have obtained your medical history, baseline bloodwork, family history and risk stratification information as well as your nutrition needs and goals.

The Be In Control package is suitable for those clients who are looking for lifestyle change and a client with medical issues in which nutrition plays an important role such as:

• High cholesterol
• Hypertension
• Diabetes
• Congestive Heart Failure
• Asthma
• Chronic Renal Failure
• Others 

This package will include your initial assessment as well as follow up visits with both the physician and dietitian to ensure the best continuity of care.  There are different options in terms of number of follow up visits based on your needs.

Each package includes:

• Initial consultation with Dietitian and Physician
• Follow ups – number depends on program length
• Constant monitoring of bloodwork and vitals
• Weekly “check ups” to monitor your progress
• Access to “online coaching” from the dietitian
• Initial body composition analysis using our State of the Art equipment
• Computer diet analysis using accurate food processor software
• Follow up body composition analysis to monitor progress
• Multidisciplinary approach to healthcare
